What is a neurological examination?

A neurological examination, also called a neurological examination, is an evaluation of a person’s nervous system that can be performed in a doctor’s office. It can be done with instruments such as a pencil light, an ophthalmoscope, a reflex hammer and a tuning fork and is usually not painful for the patient. The extent of the examination depends on many factors, such as the patient’s initial problem, age and state of health.

What is done during a neurological examination?

The following is an overview of some of the areas that may be tested and assessed during a neurological examination:

  • Mental state
  • Motor function and balance
  • Sensory examination
  • Reflexes
  • Cranial nerves
  • Gait and coordination
  • Clinical neurophysiology

A medical discipline that studies the function of the central and peripheral nervous system by recording bioelectrical activity. Tests performed include measurements of the electrical function of the brain, spinal cord, and nerves in the limbs and muscles. It can accurately define the site, type and degree of injury and detect relevant abnormalities. Clinical neurophysiology is used to diagnose diseases rather than treat them.

At ACPN, we offer the following services to our patients:

1. Electroencephalogram (EEG)

An EEG is a test that records brain activity. During the procedure, electrodes made of small metal disks with fine wires are taped to the scalp. The electrodes detect small electrical charges that result from the activity of brain cells. The charges are amplified and displayed as a graph on a computer screen. Patients are then exposed to various external stimuli, such as bright or flashing lights or sounds. The patient is asked to open and close their eyes or to hyperventilate, i.e., breathe deeply for several minutes. Electrodes transmit the resulting changes in brain wave patterns. Because movement and nervousness can alter brain wave patterns, patients usually lean against a chair or bed during the test. This test may take an hour or more depending on the doctor’s orders. There is no pain associated with the EEG.

EEG is one of the main diagnostic tests for epilepsy. It can also be useful in diagnosing other brain disorders such as brain damage related to head injuries, brain inflammation, and metabolic and degenerative disorders. It is also used to confirm brain death in patients on machines. EEG cannot measure intelligence or detect mental illness.

2. Nerve conduction studies (NCS)/electromyography (EMG)

In most cases, NCS and EMG are performed together because they complement each other.

Nerve conduction studies involve measuring the speed of conduction of an electrical impulse through a nerve.

In this test, several flat metal electrodes are attached to the skin using tape or paste. The shock-emitting electrode is placed directly over the area of the nerve, and the recording electrode is placed over the muscles controlled by that nerve. Several rapid electrical impulses are delivered to the nerve and the time it takes for the muscle to contract in response to the electrical impulse is recorded. The same nerve on the other side of the body can be studied for comparison. If it is a sensory nerve, then the signal would be monitored at a different point in its course.

NCS can take from 15 minutes to 1 hour or longer, depending on how many nerves and muscles are being studied. This study can determine if the nerve is functioning normally. This test is used to determine the functionality of peripheral nerves in the hands and feet. It can show if a nerve is pinched, and estimate the severity and location of the injury.

Muscles are constantly receiving electrical signals from nerves that are functioning properly, and in turn send out their own electrical signals. In an EMG, the electrical activity of the muscles is measured. The NCS is performed by a technician with specialized training in neurophysiology, while the EMG is performed by a neurologist who reads both parts and issues a report. In an EMG, the neurologist inserts a very thin needle into certain muscles, one at a time, to record the electrical signals from the muscles being examined. If the muscle is not getting enough impulses from the nerve, it sends signals that indicate that the muscle is not working properly. EMG results are often correlated with NCS results. This test takes 15 to 30 minutes to complete, depending on the number of areas examined.

3. Evoked potentials

Evoked potential studies measure electrical activity in the brain in response to stimulation by sight, sound, or touch. Stimuli entering the brain through each of these senses elicit tiny electrical signals. These signals propagate along nerves and, if touched, through the spinal cord to specific areas of the brain and are picked up by electrodes, amplified and visualized for interpretation by the physician.

Visual Evoked Potential (VEP) – This test can diagnose problems with the optic nerves that affect vision. Electrodes are placed along the scalp and electrical signals are recorded while watching a checkerboard pattern flash on a screen for several minutes.

Auditory Brainstem Response (ABR) – This test can diagnose the ability to hear and may indicate possible brain stem tumors or multiple sclerosis.
